For men, if you are feeling fatigued, have brain fog, losing muscle mass, and libido testosterone replacement may be for you. Testosterone injections are prescribed after an initial consultation which cost $350 and include needed labs that will be drawn at our clinic. After the labs are reviewed which is included in the initial visit cost, a 3-month prescription will be sent to the pharmacy of your choice if testosterone therapy is needed. A required follow-up appointment to check your testosterone level will be scheduled for you at the initial appointment and will be after 3 months of injections. The follow-up consultation is $250 and includes labs, and at that time another prescription for 3 months will be sent in for you.
Appointments & Cancellations
A $100 deposit is required to book. Your deposit will go toward your appointment.
Deposits on new appointments are refundable within one week of
booking– otherwise, deposits are NON-REFUNDABLE. In addition, you will only be allowed to reschedule an appointment once.
If you need to reschedule a second time, you will lose your initial deposit. A new deposit will be required to schedule the third appointment.
There is a 48-hour cancellation/reschedule policy and a 15-minute late policy. Please consider traffic and parking time to avoid being late for your session.
Be advised that giving less than 48 hours' notice will result in losing your deposit or paying 100% of the appointment to rebook. If you do not show up for your appointment, cancel the same day, or simply cannot make it within 15 minutes of your appointment time, you may charged the full cost of the appointment. When in doubt, call ahead if you think you will be late.
If you think you will be more than 5 minutes late, please text and let me know you are still attending your appointment.
If you think you will be more than 15 minutes late, please text and see if I can accommodate you; otherwise, we will reschedule your appointment.
If you do not call or show up for your appointment, you will be charged the full price. The penalty must be paid before rebooking.
Treatment Policies
Prior to any treatment, a comprehensive consultation will be provided to determine the best course of action for the client’s aesthetic goals.
Clients must complete and sign an informed consent form prior to receiving any treatment.
Trained and certified professionals will carry out all treatments.
We reserve the right to refuse treatment if it is deemed unsuitable for the client or if the client is uncooperative.
Pre-Treatment Guidelines
Avoid sun exposure or tanning beds for at least two weeks before treatment.
Before your treatment, please inform our staff about any medications, allergies, or health conditions.
For medical aesthetic patients, please come to the appointment with no makeup and clean skin. We will clean your skin again before any procedures. This step is necessary for JC and her staff to assess your skin.
Payment & Refunds
Full payment is expected at the time of service.
We accept various payment methods, including credit/debit cards and cash.
Due to the nature of our treatments, we do not offer refunds. However, if you experience issues with the service provided, please discuss them with our team.
Post-Treatment Care
Following any treatment, specific aftercare guidelines will be provided. Adherence to these guidelines is crucial for best results and to avoid complications.
Clients are urged to contact Hazelnut Aesthetics immediately for any signs of complications or unexpected reactions.
Hormone Replacement Therapy (HRT)
Only qualified medical professionals will administer and oversee HRT.
A comprehensive consultation and medical evaluation are mandatory before starting HRT.
Regular follow-up appointments will be scheduled to monitor and adjust hormone levels as necessary.
Patients will be given a lab order form to complete for hormone replacement. Following the consultation appointment and before undergoing treatment, it is important for women to have their lab tests completed. Additionally, they must have a current mammogram report to be added to their chart records.
